• FOOD & CLOTHING Staff provide emergency assistance and referrals to meet basic needs in supporting individuals and families to maintain their housing or connect with services to find housing.
• HEALTHY CALIFORNIA FAMILIES
Staff network and collaborate with community partners to offer education, resources, and referrals to underserved and under resourced families.
• CALAIM COMMUNITY SUPPORTS FOR HOUSING TRANSITION/ NAVIGATION, DEPOSITS, AND TENANCY & SUSTAINING SERVICES
Our staff team assist and support individuals and families with Medi-Cal experiencing homelessness or at risk for homelessness to secure permanent housing and supportive services to improve their overall health and quality of life.
• CALAIM ENHANCED CARE MANAGEMENT
Our staff team assist and support individuals and families with Medi-Cal who are experiencing homelessness and also have a physical or psychological disability, connecting them with needed resources and facilitating community integration.
• INTENSIVE CASE MANAGEMENT SERVICES (ICMS)
Staff support individuals and/or families experiencing homelessness to secure and/or retain permanent housing to improve overall health and quality of life. All referrals are made by LA County DHS.
• PERMANENT SUPPORTIVE HOUSING - IT’S A NEW DAY
In partnership with the City of Long Beach, our team help clients obtain stable long-term housing. In addition, supportive services are provided for individuals and families experiencing homelessness with a verifiable disability.
• COLLEGE FOCUSED RAPID REHOUSING
In partnership with CSULB and the CSU Chancellor’s Office, our team help clients obtain stable long-term housing in the community and become self sufficient.
