• FOOD, CLOTHING, HYGIENE, RENTAL ASSISTANCE & ENTITLEMENT DOCUMENTS

Our staff offer emergency assistance and referrals to help support an individual’s ability to keep their housing or to connect with services to find housing and maintain self-sufficiency.

• THE SHOWER OF HOPE

Through our partnership with Trinity Church, LSSSC hosts The Shower of Hope to provide our clients with access to shower services. Our staff provide outreach to individuals who utilize this service including employment assistance through the Workforce Development Center.

• PEER SUPPORT TRAINING & DEVELOPMENT PROGRAM

Through this peer support specialist program, our staff recruit, train, and place individuals with lived experience to walk alongside individuals and families in the public mental health and recovery systems of care.

• CalAIM PROGRAM

CalAIM Community Supports program serves members experiencing homelessness, at-risk of homelessness, formerly incarcerated. or low-income individuals/families in the Inland Empire to secure and maintain independent permanent housing. CalAIM Community Supports also focuses on housing navigation and sustainability.

• Case Management (drop-ins welcome): assistance with documentation replacement, referrals to other CalAIM services including medical case management, medically tailored meals, and day habilitation. Provides refferal to potential permanent housing opportunities, assists with housing search, deposit, and furniture assistance, and provides case management for 6-12 months after securing housing.

• House Match: Ensure client meets eligibility criteria, submit required documentation, assist in housing navigation and unit viewings, application fees, and security deposit.

• Housing Tenancy & Sustainability: Provides housing deposit, assist with lease signing, 1st month rent, and housing essentials. Provides resources for sustainability needed, and monthly income visits for ongoing support.
